How Did This Accident Occur?

It’s still early in the investigations, so many unanswered questions still remain. Was this due to a structural failure on a storage tank? Did a truck operator dump out the tar without seeing the worker? Did the worker fall into the tar? Was there equipment failure or some other catastrophic failure leading to the accident? It’s going to take extensive investigations to find out for sure.

What’s important to recognize, though, is those answers rarely come quickly. OSHA may be looking into things, but it can taken them several months or even more than a year to finalize their reports. Even then, their efforts don’t prioritize the needs of a family, which is why many find themselves left out of the loop in the meantime. Getting answers sooner could mean bringing in independent professionals who can work in the family’s interests rather than focusing on a narrow set of priorities like authorities tend to do.

Regardless of what happened here, does anyone know more about the site where this occurred? Is it generally a safe place to work, or have there been issues before? Let me know in the comments.
