How Did This Accident Occur?

A two-car accident at an intersection like this generally means someone failed to yield the right-of-way. But people tend to think that automatically means a driver made a mistake. That’s not really true. It’s common, of course, considering things like cellphone use and speeding. However, investigations also need to rule out possibilities like a medical emergency, faulty brakes, tire failure, malfunctioning traffic signals, etc. Even if some of these seem unlikely, I’ve seen examples of each of them, and they almost never came up during the initial accident investigations. That sort of thing usually requires more thorough independent investigations to rule out or confirm. Hopefully steps are taken here to see that happens if it hasn’t already.
