Horseshoe Bay, TX — January 12, 2023, Lucas Spaw was fatally injured as the result of a work accident at around noon at the Summit Rock Golf Course.

According to preliminary information about the accident provided by the Occupational Safety and Health Administration, the incident happened at 384 Blazing Star, which is in the Summit Rock Golf Course Community north of Highway 71.

Investigators said in their statements that 42-year-old Lucas Spaw was working to pull wire while on a scaffold. The reports claim that the scaffolding had no fall protection, no guardrails, and no outriggers. While working, the scaffold apparently tipped over, causing Spaw to fall and suffer injuries which ultimately proved fatal.

The OSHA report lists 7 alleged rule violations, all of which were labeled as “Serious.” There was also a listed recommended penalty of $14,735. The current status of the investigation is open, and additional details are pending further investigations.
