How Did This Accident Occur?

Experience has taught me not to reach any conclusions about how an accident happened until more facts are available. That means waiting until the investigation is complete, and there are no more questions about what happened.

Will investigators look beyond typical factors like driver error or fatigue? Will they consider plausible scenarios like a software failure or throttle issue in one of the vehicles? I hope they do, but authorities cannot provide the answers the families affected by this crash need, then perhaps an independent investigation will be necessary.
