Authorities indicate that on May 23, a collision occurred between an SUV and a semi-truck on Highway 412, close to 177the West Avenue in Tulsa. According to reports, the SUV rear-ended the semi-truck sometime after 1:00 in the afternoon.
Investigators have not determined what caused the crash but report that the SUV driver, 38-year-old Jeanine Proctor, sustained life-threatening injuries in the crash and needed to be extricated from the vehicle. Proctor was transported to an area hospital for treatment and did not survive. No other injuries were reported.
