When I hear about a serious crash like the one reported on Interstate 495, where a vehicle left the roadway for reasons that have yet to be determined, I immediately think about how critical it is to uncover the why behind the incident. Two people were seriously injured, and the vague description that the vehicle “lost control” leaves many important questions unanswered. In situations like this, I always come back to the same three questions that guide any thorough investigation: Did the authorities thoroughly investigate the crash? Has anyone looked into the possibility that a vehicle defect caused the crash? And has all the electronic data relating to the crash been collected?
First, the completeness of the investigation is key. Crashes where a vehicle leaves the roadway without any known external cause deserve especially close scrutiny. That includes analyzing the scene for tire marks, mechanical debris, or road surface irregularities, as well as gathering witness statements if any are available. But this level of investigation isn’t guaranteed. Depending on the jurisdiction and available resources, some crash scenes are only briefly documented, particularly when there’s no sign of criminal activity or involvement of another vehicle. That means potential evidence—whether physical clues on the road or behavioral signs from the occupants—might never be examined.
Second, vehicle defects are a possibility that simply doesn’t get the attention it should in most single-vehicle crashes. It’s easy to assume driver error, but we also have to consider whether something inside the vehicle failed. A malfunction in the steering system, unexpected brake failure, or even an issue with the vehicle’s electronic stability control could all lead to a complete loss of control. These kinds of problems are rarely visible without a detailed mechanical inspection, and unfortunately, in many cases, no one conducts that level of review. That’s a missed opportunity to identify whether a defect contributed not just to this crash, but possibly to others involving the same make or model.
The third question—whether all electronic data has been gathered—is just as critical. Most newer vehicles are equipped with event data recorders that store essential crash-related information, such as vehicle speed, throttle position, braking activity, and steering input. This data can provide an unbiased account of what the vehicle was doing in the moments before the crash. Additionally, if either occupant had a phone running a navigation app or if there were surveillance cameras near the area, that information could help clarify the sequence of events. These data sources can be the difference between a case filled with uncertainty and one where we can say with confidence what occurred and why.
Every crash that results in serious injury leaves behind not just wreckage, but questions—questions that deserve answers. And the only way to get those answers is to ensure that the investigation is detailed, open to all possibilities including vehicle failure, and supported by all available data. That’s not just a procedural necessity—it’s a matter of justice and accountability for those whose lives have been upended.
