Accident Report

August 27, 2025, Bradley Hutton, Bradley Wrightsman, and John Nelson were killed in a work accident which happened at around 3:52 p.m. at 22 Creeks Edge.

Initial details about the incident say it happened near Westwood Lake east of Trinity.

Preliminary information says that Bradley Hutton, Bradley Wrightsman, and John Nelson were working on sewage treatment at the time. It appears the three workers each went into a manhole where a sewage line had broken loose. Trinity County Sheriff’s officials say it’s believed gases from the sewage caused all three men to lose consciousness. Ultimately, Bradley Hutton, Bradley Wrightsman, and John Nelson were killed.

It’s unclear at this time if OSHA is considering citations or penalties in relation to the accident. Investigations appear to be ongoing.

How Did This Accident Occur?

To say that this all happened due to toxic gas buildup wouldn’t really get to the heart of what happened here. The workers’ families deserve to know what kind of conditions the victims were working in, what kind of equipment, training, and supervision they had, and how all of this could have been avoided. One can hope authorities have provided those answers behind the scenes, but I get calls from families all the time who find themselves asking the same questions that they had on day one months after an accident.

At the end of the day, Texas has the most complex work fatality laws in the whole country. Navigating that legal maze starts with having all the facts. A lot of people find that OSHA focuses on their own priorities, so they need independent investigators looking at the details separately. Doing so can help ensure both that the families see a proper resolution and also to help ensure this sort of thing never happens again.
