Houston, TX — April 1, 2022, Alexander Uzcatequi Canizelez was killed following a fall accident on a work site along the Southwest Freeway.
Preliminary details provided by the Occupational Safety and Health Administration indicated the incident happened at approximately 8:43 a.m. at 8313 Southwest Freeway on a job site for Gulf Coast Flooring & Services.

It appears that 42-year-old Alexander Uzcatequi Canizelez was working with machinery to remove foam from a commercial building rooftop. While doing so, investigators say that Alexander Uzcatequi Canizelez tripped and fell, causing him to fall through a skylight. Alexander Uzcatequi Canizelez fell into the building and landed on the first floor.
Alexander Uzcatequi Canizelez died as a result of the accident. No others were said to be hurt. It can’t be confirmed right now what the results of the OSHA investigations are, but their statement claim there was at least one “willful” violation for which they recommended a penalty of $101,519. No further information is available at this time.
