Fort Worth, TX — December 20, 2024, Lee Chu was fatally injured as the result of a workplace accident which occurred at 7:15 a.m. along Blue Mound Road.

OSHA released an inspection summary about the accident, which reportedly happened between Meacham Boulevard and Loop 820.

According to officials, 49-year-old Lee Chu was in the vicinity of a materials rack as a forklift passed by. The forklift apparently struck material on the rack, causing it to rotate and strike Lee Chu in the head. EMS took Lee Chu to a hospital where he later died.

No other injuries were reported. OSHA listed three alleged “serious” violation citations as well as a recommended penalty. No further information is available.

It’s obviously concerning to see OSHA alleging multiple violations here. However, that doesn’t mean this all will be open-and-shut. Texas has the most complex work fatality laws in the country. Despite what many think, there isn’t always an automatic process available to the worker’s family to get the benefits they need. Sometimes, that takes navigate a significant legal maze, which OSHA is not going to go out of their way to help accomplish.

As such, families who lose a loved one on the job generally have to have outside help navigating those potential paths forward if they’re going to get the best outcomes. Texas work fatality law may be uniquely complex, but the right experience, evidence, and tools make overcoming that obstacle achievable.

So even if the alleged violations reported by OSHA are accurate, there could still be a lot more to this story.
