San Augustine, TX — December 5, 2022, Jeremy Martin, Hershel Sullivan were killed due to a work accident at around 7:40 p.m. off County Road 131.
The Occupational Safety and Health Administration released an investigation summary following preliminary investigations into the fatal accident. Their statements say that the company their investigation was focused on was Jaguar Energy Services.

The details released by investigators say that 23-year-old Jeremy Martin and 69-year-old Hershel Sullivan were working with a gas well. Details suggest they were working to “remove a Christmas tree and install a blowout preventer.” While doing so, it appears that a pipe burst after opening the valve on the Christmas tree. This caused the pipe to “swing wildly back and forth,” according to officials,
At some point during this failure, Martin and Sullivan both sustained injuries which ultimately proved fatal. Information says two other workers were hurt in the accident and since recovered. OSHA officials identified one alleged rule violation in their summary as well as a recommended penalty of just under $11,000. The status of the case at this time is listed as “Open.” No further information can be confirmed pending investigations.
