How Did This Accident Occur?

I understand that most people who read about this crash would be tempted to blame it on the first driver and move on. After all, he’s the one who crashed into another truck that was stopped in traffic, right? That’s what we’ve been told, but I’ve seen enough commercial vehicle crashes in my career to know there are still more questions to be asked.

What if that first truck had a problem with its brakes that kept it from stopping with traffic? Or an issue with its throttle that caused unexpected acceleration? Did the stopped truck have working brake lights, so the vehicles behind it would know it was stopped?

These questions must be answered by investigators so those affected by this crash will know those it unfolded.
