Commentary by Attorney Michael Grossman

An accident like can sometimes be a lot more challenging than people realize, since loading and unloading cargo can involve multiple different companies. Sometimes, the people load things improperly, resulting in the people at the destination unloading it and getting hurt. Sometimes, equipment used to secure the cargo is used wrong or has some kind of defect. There may be other issues with the working environment that made something like this more likely to occur.

With these multiple moving parts, typical, surface-level investigations may let something important slip through the cracks. Perhaps OSHA was able to get the victims’ families clear answers, but it’s possible doing so would require more thorough independent investigations. Were those steps ever taken?
