Accident Report
April 21, 2026, Carlos Fajardo lost his life due to a car accident just before 9:45 p.m. along West Seminary Drive.
According to authorities, 36-year-old Carlos Fajardo was traveling in a westbound Mitsubishi Outlander on West Seminary Drive at the railroad crossing west of the Town Center Drive intersection when the accident took place. Officials indicate that, for reasons yet to be confirmed, the Outlander allegedly attempted a lane change at an unsafe time. A sideswipe collision consequently occurred between it and a westbound Honda Civic.
Fajardo reportedly sustained serious injuries over the course of the accident. Additional details pertaining to this incident are not available at this point in time. The investigation is currently ongoing.
How Did This Accident Occur?
I can understand why it would be tempting for some to assume that this accident was caused by driver error. However, over the last thirty years I’ve been in this line of business, I’ve learned not to jump to conclusions regardless of how things appear on the surface. Why? Because, I’ve seen plenty of cases in which evidence of less likely causes ended up being unearthed by the investigation.
That’s not to imply that I know more about this specific accident than anyone else outside of the investigation would or should. To be honest, as far as I am aware, driver error—whether it was caused by fatigue, distraction, impairment, et cetera—remains on the table as a possible cause. However, I would like to point out that, hypothetically, something could have gone wrong with the Outlander to cause the wreck rather than driver error.
For example, what it the sideswipe collision was a result of not an attempted lane change, but a steering mechanism issue? I understand that it’s not likely, but neither is it outside of the realm of possibilities. Additionally, I would be interested to know whether or not the safety systems in the Outlander—like the airbags and the seatbelt—worked the way they were supposed to. Why? Because if not, then it is possible that the victim’s injuries ended up being more sever than they might otherwise have been.
Hopefully investigators—whether the authorities or a third party—get an in-depth vehicle inspection done on the Mitsubishi. That way any underlying mechanical malfunctions or product defects that played a role in the wreck can be brought to light. After all, the people affected by this accident deserve a clear and detailed understanding as to how and why it happened. Vague assuptions based on surface-level investigation won’t cut it.
