How Did This Accident Occur?

Generally speaking, the first factor investigators look into after a pedestrian accident is who had the right-of-way. But that doesn’t always tell the whole story. For example, I’ve seen accidents throughout my career where a particular stretch of road or an intersection was dangerous for pedestrians. It could be there’s a lack of lighting, no traffic control devices, poor markings on the road, or failure to improve areas with a known history of accidents. Maybe that isn’t what happened here, but any decent accident investigation would be able to rule that out. Did authorities rule that out here?
