September 2, 2025, Carlos Fierro Alvarado was killed following a work accident that happened along Aylesbury Road.

OSHA released few details surrounding the accident. It reportedly happened in a neighborhood west of the McLeroy Boulevard and Knowles Drive traffic circle.

The nature of the accident is unclear, and OSHA did not specify what occurred in the incident. Ultimately, 41-year-old Carlos Fierro Alvarado was pronounced deceased.

How Did This Accident Occur?

There’s no way to responsibly draw any conclusions about a workplace fatality accident with so few details available. The last thing I’d want anyone doing is jumping to conclusions. But months after an accident, people generally expect OSHA to have some answers. Are they just being tight-lipped with the public, or do they really not have any idea what happened here? It would surprise people how often it turned out to be the latter.

To be clear, authorities may have plenty of information about what happened here which they’ve shared with the victim’s loved ones. I’ve had families come to me with work accidents where authorities did a fine job, and independent investigations helped reaffirm that authorities didn’t miss anything important.

A lot of the time, though, families are just as in the dark as anyone else. OSHA can move very slowly, and their efforts don’t always align with the needs of a family. That means a worker’s loved ones might not have any more answers weeks or months after the accident than they had on day one. That’s why it’s understandable that so many seek out independent investigators in the first place. I know if I lost a loved one to a workplace accident, I’d want answers as soon as possible.
