A person was killed late Monday night when a semi-truck crossed the median and struck a passenger vehicle head-on on Interstate 74 in Piatt County, Illinois.

According to Illinois State Police, the crash occurred just before midnight around milepost 167 near Mansfield. Reports indicate that the semi-truck was traveling eastbound when it allegedly crossed the median into the westbound lanes, where it collided with a passenger vehicle. One person in the vehicle was pronounced dead at the scene. The Piatt County Sheriff’s Department and Coroner’s Office are assisting State Police with the ongoing investigation.

Whenever I hear about a semi-truck crossing a highway median and hitting another vehicle head-on, I think about how many things have to go wrong for that to happen. In my experience, cases like this often involve a combination of human error and mechanical issues.

Could Driver Fatigue or Inattention Have Been a Factor?

A semi-truck drifting or swerving across a highway median late at night raises immediate questions about the driver’s condition. With the crash occurring just before midnight, fatigue could have impaired the driver’s alertness or reaction time. Investigators will likely examine the driver’s hours-of-service logs, recent rest periods, and in-cab data to determine whether the driver was fully alert at the time of the crash.

Distraction is also a possibility. Reviewing cell phone records, dash cam footage, and GPS data may help establish whether the driver lost focus in the moments leading up to the collision.

Could Mechanical Issues Have Caused the Truck to Leave Its Lane?

Beyond driver behavior, it’s important to look at whether the truck was functioning properly. A mechanical failure—such as a steering issue, brake malfunction, or tire blowout—could cause a sudden loss of control, especially at highway speeds. Investigators should review maintenance records, inspect critical components, and examine whether the semi-truck passed its most recent safety inspection. If the truck experienced a failure that contributed to it crossing the median, that could point to deeper problems with how the vehicle was maintained or potentially a defect in the parts themselves.

What Types of Evidence Should Be Examined?

To determine exactly what happened, investigators will need more than witness statements. The truck’s Engine Control Module (ECM) can show speed, throttle, and braking activity in the seconds before the crash. Physical evidence from the roadway—such as tire marks, debris patterns, and vehicle positioning—will also help reconstruct the event.

If any onboard cameras or nearby traffic surveillance footage is available, that footage could clarify how the truck moved across the median and whether the driver attempted to correct course.

Why Ask These Questions?

When a semi-truck crosses the median and causes a fatal head-on crash, there are critical questions that must be answered—about both the driver and the vehicle. From my experience, a full investigation is the only way to get those answers for those affected by the accident.
