According to official reports, a vehicle with two occupants was traveling on the service road of Hwy. 191 in the eastbound lanes, when for unknown reasons it struck the rear of a Dodge Ram pickup which was driving in front of it, the impact of which caused the pickup to drive the cable barrier where it then entered the main lanes of Hwy. 191. The other vehicle lost control and crashed through a barbed wire fence before stopping on a gravel parking area.

When first responders arrived on the scene, they found that both occupants of the second vehicle had sustained critical injuries and the Dodge driver suffered minor injuries, and they were transported to the hospital for treatment.
