A serious multi-vehicle crash occurred early Friday morning on US 60 near Mill Avenue in Tempe, Arizona. According to the Arizona Department of Public Safety, the incident happened around 4:20 a.m. when a Bob’s Barricades attenuator truck, which was traveling in the left lane while a worker collected traffic barricades, was reportedly rear-ended by a pickup truck. The impact caused the pickup to spin and strike a passing box truck. Five people were transported to hospitals, and the pickup driver reportedly suffered serious injuries.

In crashes like this, I look closely at whether the commercial vehicles involved were properly marked and operating safely for the conditions at that time of day.

Were Proper Hazard Signals in Place?

When commercial vehicles like attenuator trucks operate on active roadways, especially during low-light early morning hours, it’s essential they are clearly visible to other drivers. Investigators will likely examine whether the attenuator truck’s hazard lights, arrow board, and other safety equipment were fully functioning and activated. The position of the truck in the left lane and how clearly it was marked may be central to determining how the crash occurred.

Could Mechanical Issues Have Played a Role?

Given that the pickup truck reportedly collided directly with the attenuator vehicle, it’s worth considering whether brake failure, worn tires, or another mechanical problem contributed to the driver’s inability to stop in time. A thorough inspection of the pickup’s systems, particularly its braking components, may help clarify if a defect was a factor.

What Evidence Should Be Collected?

Key pieces of evidence will likely include dash cam footage from the commercial vehicles, ECM (Engine Control Module) data from the pickup truck, and photos of the scene showing vehicle positions and damage. Investigators may also review whether the attenuator truck had all required safety lighting and reflective markings active at the time of the crash.

Why Investigating Every Detail Matters

When a commercial vehicle is involved in a crash during active operations, it’s crucial to ensure that all safety protocols were followed and to understand whether operational error or mechanical failure played a role. A thorough investigation will help identify exactly what went wrong and why.
