How Did This Accident Occur?

When vehicles going the opposite direction collide, it’s generally because someone lost control and went over the center line. But there’s a big difference between someone drifting out of their lane because they were on their phone, for example, and a vehicle experiencing unexpected issues due to a mechanical failure. Distinguishing between these things can take far more thorough investigations than people realize. Authorities aren’t always equipped to inspect for vehicle defects, so if they don’t hand it off to independent specialists, important information could slip through the cracks. Were steps ever taken here to make sure all possible factors got the attention they needed?
