Houston, TX — June 17, 2024, Walter Elias-Villalta was killed as the result of a work accident around 3:20 p.m. along Butler Boulevard.
According to officials with OSHA, the incident took place just off the corner of Butler and Cambridge Street.
It appears that 39-year-old Walter Elias-Villalta was working on a scaffold at the time. Flooring reportedly gave way, causing Walter Elias-Villalta to fall and suffer fatal injuries. No other injuries were reported.
The OSHA statement on the accident listed three alleged “serious” violation citations. It also included a recommended penalty. Additional details are unavailable.

When people see OSHA report serious alleged violations, they figure the case must be open-and-shut from there. However, there’s a lot more to fatal work accidents in Texas than just issuing fines through an OSHA investigation. Despite what many believe, there isn’t always some automatic process for getting families the help they deserve after an accident. They often have to put a story together themselves, and that can require more evidence than the typical OSHA investigation covers. So even if the allegations of serious violations here are true, there may be a lot more to this story before the victim’s loved ones see a fair resolution.
