Accident Report

Fort Worth, TX — August 18, 2026, Tyler Triplett, Randi Holshouser and another person were killed and two others were injured in a wrong-way accident at about 1:20 a.m. on Airport Freeway/State Highway 121.

Authorities said a 2016 Jeep Wrangler was heading east in the westbound lanes of Airport Freeway when it clipped a 2026 Honda Civic before colliding head-on with a 2016 Chrysler 200 near North Riverside Drive.

Two women in the Chrysler — 32-year-old Tyler Danielle Triplett and her mother, Randi Holshouser, 64 — and the Jeep driver were pronounced dead at the scene of the crash, according to authorities.

Two other people in the Chrysler, including Tyler Triplett’s 11-year-old daughter, were hospitalized with serious injuries after the crash, authorities said.

The three people in the other vehicle were not hurt, according to authorities.

Authorities have not released any additional information about the Tarrant County crash at this time. The accident is still under investigation.

How Did This Accident Occur?

Typically a wrong-way crash like this is attributed to an alleged drunk driver. So far, authorities have not mentioned alcohol as a factor in this crash, so it appears they are considering other possible causes. Maybe the Jeep driver had a medical emergency. Maybe the driver was fighting with a mechanical issue like a balky steering mechanism. Maybe the signage was missing or damaged where the Jeep entered Airport Freeway.

That last suggestion may seem far-fetched, but I once handled a case involving a crash that happened because a construction crew failed to put a stop sign back after completing work at an intersection.

I’m not claiming that is what happened here, but it is a possibility that investigators should consider as they work to figure out what caused this crash.
