Boone County, IN — November 14, 2025, Specialist Terry Frye was killed and three others were injured in a truck accident at 12:15 p.m. on I-65.

Preliminary information about the crash say that the accident happened on southbound I-65 in the area of the I-865 interchange, north of Indianapolis.

According to officials, 23-year-old Terry Frye, a Specialist in the Indiana National Guard, was ina National Guard Humvee along with three other service members. Somehow, a multi-vehicle crash occurred involving the Humvee, a car-hauler, and an SUV.

Due to the accident, Terry Frye was killed. The three others in the Humvee reportedly were seriously injured. It’s unclear if there were other injuries. The cause of the crash remains under investigation.

A fatal accident can be challenging enough without it involving multiple large vehicles. A crash scene like that can be chaotic, and without proper investigations, important details can slip through the cracks. That’s why it can be important to consider some key questions early on.

  1. Are investigators dedicating the time, equipment, and personnel needed for a proper accident reconstruction?
  2. Have the vehicles been preserved so they can be inspected for defects or other mechanical issues?
  3. Are authorities securing electronic evidence, including vehicle data, hours of service logs, cell phone records, and video from any cameras on the vehicles or at nearby businesses or residences?
