How Did This Accident Occur?

A lot of people jump to the conclusion that a single-vehicle crash this early in the morning must be the result of a driver making a mistake. That’s common, especially considering many drivers are fatigued early in the morning. But there are many reasons a crash like this can happen without the driver doing anything wrong. A tire blowout, for example, could be caused by objects in the road, unscrupulous repair shops, or manufacturing defects. Is that something authorities have looked into here? If not, I’d be curious to know what other important factors they may be overlooking.

If anyone passed by the accident scene, did you see anything that stood out? Let me know in the comments.
