How Did This Accident Occur?
While I can’t comment on what specifically led to this accident, my experience tells me that a lot of machinery related incidents on the job site involve a lack of safety procedures or a lack of proper tools. However, it’s sometimes the result of worn down or malfunctioning machinery. That means it’s important to inspect the machine for any kind of flaws which could trace back to maintenance issues or potential manufacturing defects the workers and their employer had no way of knowing about. If that’s ultimately ruled out, then investigations can focus on potential issues with the job site’s procedures. If investigators do find a flaw or malfunction, they can keep digging to pinpoint the origin of that flaw.
