Accident Report
January 18, 2026, Ricardo Urbano was killed as the result of a work accident that happened off Highway 158.
Preliminary information about the accident says it happened at a well site off the southwest corner of TX-158 and TX-137, between Garden City and Midland.
According to OSHA, 38-year-old Ricardo Urbano was at a Oil and Gas work site when an incident occurred. As a result, Ricardo Urbano sustained fatal injuries. It’s unclear if there were any other injuries.
The cause of the accident remains under investigation at this time.
How Did This Accident Occur?
An accident at a well site could be anything from a vehicle accident to falling parts to malfunctioning machinery to a medical emergency. It’s unfortunately common for OSHA reports to contain little to no information like this. My concern is that they may be equally sparse with details with the worker’s loved ones. A lot of the calls I get from families who lost a loved one to a work accident come months after the accident. They’re stuck asking the same questions they had on day one, still with no answers.
The unfortunate reality is that OSHA has their own priorities, and they don’t necessarily align with the needs of a family. Meanwhile, families are left to navigate the most complex work fatality laws in the whole country where there might not be any guaranteed solution. So, it’s important that folks consider their options and pursue whatever avenues are available to them to get answers. If OSHA is an open book and can provide satisfactory answers, that’s great. But if getting answers requires more thorough independent investigations, that’s not at all unusual.
