Houston, TX — February 5, 2025, Rafael Vega Cabrales was killed as the result of a work accident around noon off Bissonnet Street.
The initial OSHA summary on the accident shows it happened in the area of the Stoneriver Apartments at the Bissonnet Street and Braeswood Boulevard split.
Authorities reported that 40-year-old Rafael Vega Cabrales was working to replace piping in an excavated trench. While doing so, the wall caved in, ultimately causing Rafael Vega Cabrales to suffer fatal injuries. There did not appear to be any other injuries.
OSHA officials allege that “there was no protective system in use” at the time. They listed seven alleged “serious” violation citations and a recommended penalty.

If the allegations here prove to be true, I have to wonder what steps are being taken to investigate beyond just this accident. Texas has the most complex work fatality laws in the country, and it’s important a worker’s family has as much information in their hands as possible to navigate those laws. That might lead to decisions that were made before the victim ever clocked in for work on this day.
Did this involve a company with a history of violations? Have other workers been hurt or killed due to lack of safety equipment? Have employees made complaints that went ignored? Was there not a supervisor at the site, or was the employer forcing workers into dangerous situations?
Seeing that a family gets answers and has an opportunity to get a just resolution is one thing. It’s another thing entirely to make sure other families won’t have to go through the same hardships. An OSHA investigation alone might not be enough to set all of the wrongs right.
