How Did This Accident Occur?
Without knowing more specific information about this accident, I can’t say what exactly led to this fatal incident. It could be that workers weren’t following proper safety procedures. Perhaps a supervisor pressured the workers into doing something irresponsible for the sake of time or cutting costs. Maybe this all traces back to equipment failure due to poor maintenance or some unforeseeable defect. Ultimately, that’s for the evidence to say.
My hope is that someone worked to get that evidence to the victim’s loved ones early in the investigations. Too often, I speak to families who trusted authorities to get them answers only to find themselves months down the line still asking the same old questions. Making up for that usually means starting from square one and getting professionals to dig into the evidence and put the pieces of the puzzle together independent from the OSHA investigations. After all, OSHA tends to focus on their own priorities. For families to get a fair and just resolution, they usually need someone looking out for their priorities, as well.
