How Did This Accident Occur?

Two vehicles going opposite directions and crashing at a traffic light usually happens because someone was attempting to turn through the intersection. However, people often immediately assume the person turning is the one who messed up. Things aren’t always so simple.

What if they had a protected green arrow? What if the light malfunctioned? What if the oncoming driver was speeding or looking at their phone? What if an automatic driving system glitched out and caused a driver to lose control?

At the end of the day, it’s not about what’s likely or not. It’s about what the evidence says. Jumping to conclusions simply won’t do any good. I know I have plenty of examples throughout my career that are proof of this. Does anyone else have examples they or someone they know experienced first-hand?
