How Did This Accident Occur?

I can’t answer this question any more than the sparse preliminary OSHA statements can. What’s most concerning to me is if the family is getting any answers behind the scenes. It would shock folks how often families call me weeks or months after a fatal work accident, and they have the same unanswered questions that they had on day one. The unfortunate reality is authorities tend to focus on their own priorities. The needs of the worker’s family usually get put on the back burner. Sometimes, they slip through the cracks altogether.

So, regardless of what happened here, my hope is that professionals have taken the time not just to look for blame but to preserve all of the facts. In my experience, families stand the best chance of getting a fair resolution when there are professionals working to get them clear, thorough answers about the accident. The sooner that happens, the better.
