Dallas, TX — August 7, 2025, Leonel Velasquez was killed as the result of a work accident which occurred off Swiss Avenue.

People tend to look at OSHA as the agency that takes care of everything after a work accident. In reality, they have pretty narrow priorities: look for violations and issue appropriate penalties. Beyond that, getting answers for the worker’s family and helping them navigate Texas work fatality law isn’t really on their radar. So while one might hope that a victim’s family has more answers behind the scenes than the public has, that’s not often the case. That tends to happen only when steps were taken early on to have independent investigators dig into the details for the family. Maybe OSHA has been more forthcoming with the family affected by this accident, but it’s not something I’d ever take for granted.
