March 24, 2026, Lee Wane was killed as the result of a work accident that took place around 11:30 a.m. on Mestina Street.

In initial statements on the accident, it reportedly took place in a manhole off the corner of Mestina Street and Tancahua Street.

According to officials, 53-year-old Lee Wane was reportedly doing electrical work in a manhole along Mestina Street. Reports say that Lee Wane may have been electrocuted while working, but exact circumstances are unconfirmed.

An OSHA investigation appears to be ongoing.

How Did This Accident Occur?

Without further information, I don’t want to speculate about what caused this accident. It could be there were hazardous conditions at the job site, or maybe the victim wasn’t provided with adequate safety gear by their employer. Maybe this all was simply unavoidable. What matters is making sure the victim’s family gets answers, which is generally what folks assume OSHA is there for. That’s unfortunately not always true.

In reality, OSHA’s priorities are looking for possible violations and recommending penalties for those violations. When it comes to getting answers for a worker’s family, that tends to be an afterthought. That’s why I often hear from families months after an accident still trying to get answers to questions they were asking on day one. Independent investigations can help fill in those gaps when OSHA drops the ball. Hopefully, though, that won’t ultimately be necessary here.
