How Did This Accident Occur?
The relatively few specifics listed in the OSHA report only create room for speculation, which is something I don’t like to do. A death on the job site with a crane could mean there were issues related to misuse of machinery. Or, maybe the machinery itself malfunctioned. Maybe the machine wasn’t directly involved in the accident. I’ve even had a case where a supervisor getting into a fist fight next to a machine’s controls led to the death of a worker. At the end of the day, an OSHA report like this doesn’t automatically mean someone did or didn’t do anything wrong. My concern is just if anyone continued digging beyond what I’m seeing in the online reports so the family could get some clear answers.
