Accident Report

June 28, 2026, Koreana Perkins was injured due to a rollover car accident shortly after 2:30 a.m. along Mesa Drive.

According to authorities, 25-year-old Koreana Perkins was traveling in a southeast bound Buick Envista on Mesa Drive at the railroad crossing in the vicinity south of the Furay Avenue intersection when the accident took place. Officials indicate that, for as yet unknown reasons, the Envista was allegedly traveling at unsafe speeds. It was consequently involved in a single-vehicle collision in which it apparently overturned.

Perkins reportedly sustained serious injuries over the course of the accident. Additional details pertaining to this incident are not available at this point in time. The investigation is currently ongoing.

How Did This Accident Occur?

When people hear about single-car rollover accidents involving alleged unsafe speeds, many are quick to assume that they must have been a result of the ubiquitous “driver error”. That’s a safe bet, considering the fact that human error plays a role in the majority of car accidents; however, I’m not ready to settle on that conclusion without more information. Why? Because over the last thirty years of my career I have seen plenty of similar cases in which thorough investigation managed to uncover evidence of less likely causes. Sometimes things turn out to have been different than they initially appeared on the surface.

To be completely clear, I’m not implying that I have more information about this specific accident than is already publicly available. As far as I am aware, driver error—whether it was a result of fatigue, distraction, impairment, or something else entirely—remains on the table as a possible cause. However, I would like to point out that, hypothetically, the Envista’s excessive speeds and subsequent collision could have been a result of something gone wrong with the vehicle itself rather than driver error. For example, what if it was dealing with brake failure? Or maybe its steering mechanism was having issues? Have tire blowouts or throttle problems been considered? I do understand that these scenarios are not particularly likely, but that does not mean they should not at least be looked into.

Beyond that, there is the fact that the vehicle overturned. Most people are not aware of this, but the rollover tests that car manufacturers are required to pass do not actually do a very good job of simulating real-world scenarios. I would be interested to know whether or not a design flaw with the vehicle had a hand in its rollover. Why? Because the victim’s injuries might not have been as severe had it managed to remain safely upright.

Hopefully investigators—whether the authorities or, if need be, a third party—get an in-depth vehicle inspection done on the Buick. That is what will be able to pinpoint any mechanical malfunctions, product defects, or design flaws that contributed to the accident or its outcome rather than allowing them to fly under the radar unnoticed. After all, the people affected by this accident deserve to be given a clear and detailed understanding as to how and why it happened. Surface-level investigation just won’t cut it.
