How Did This Accident Occur?

Trench collapses always bring up immediate concerns about safety precautions. Were the walls secured at all? Was collapse prevention installed correctly? Was safety equipment installed, but it failed for some reason? Was there adequate supervision on this site, including regular inspections? Have workers on this site or at this company complained about working conditions in the past?

People often write off work accidents as momentary mistakes, but I’ve seen throughout my career how often they’re the result of continuous failure to ensure workers are safe and protected on the job site. That’s something OSHA may be looking into, but their efforts can take months, and they might not even answer all of the questions the workers’ families will no doubt have.

That’s why a lot of work accidents fare better when multiple professionals are investigating it from different angles. While OSHA may focus solely on looking for possible rule violations to address, independent investigators can focus on asking questions that demand more prompt answers. This sort of approach can often help families move forward, whatever path forward that may be.
