As straight-forward as these reports seem—authorities say that someone disobeyed a traffic signal—it’s always important to have the full story. Even something unlikely, such as a mechanical defect, should at least get attention from well-equipped investigators. That, the victims and families can make sure there are no gaps in the story. I’ve been doing this for 30 years, and I’ve almost always found it to be more effective than just focusing only on the most obvious details coming from a crash report.
