Accident Report
March 23, 2026, Jeremiah Smith lost his life due to a two-truck accident shortly after 7:15 a.m. along State Highway 94.
According to authorities, two cement trucks—one occupied by 48-year-old Jeremiah Smith—were traveling in opposing directions on S.H. 94 in the vicinity east of the S.H. 564 intersection when the accident took place. Officials indicate that, for as yet unknown reasons, the two trucks apparently made contact with each other. The cement truck that Smith was in apparently veered off of the roadway and overturned into a creek. The other truck also left the roadway, overturning on dry ground, according to reports.
Smith reportedly sustained fatal injuries over the course of the accident. Additional details pertaining to this incident are not available at this point in time. The investigation is currently ongoing.
How Did This Accident Occur?
Over the last three decades, I’ve had the opportunity to analyze hundreds of different truck accidents. This one—which involved two commercial vehicles—sparks my interest. I’m interested to know how the authorities are handling the investigation. Are they getting tunnel vision and focusing too closely on the evidence at the physical scene? Or are they peeling back the layers of the wreck and digging deep to find any underlying causes that might otherwise fly under the radar?
At this point, reports don’t state what happened to cause the collision. Did one truck veer left of center? Was it a head-on collision? Or a sideswipe? Either way, both trucks ended up overturning off of the roadway. I’d be interested to know what the trucks’ ECMs say, not to mention what driver schedules, inspection logs, and maintenance history would divulge. If it turns out that a preventable mechanical issue with one of the trucks caused the wreck, but was swept under the rug to save the company time and money, then the people who made those decisions hold a piece of the responsibility for the outcome of this collision.
Hopefully the authorities are asking the right questions. If it seems like their work is falling short in any way, a third party investigation can always be called in to take up that slack.
