Houston, TX — June 7, 2025, Isaac English was killed as the result of a work accident which took place off of Ardmore Street.
Investigators from the Occupational Safety and Health Administration released preliminary information about their inspections. The location of the accident was near Ardmore Street and Yellowstone Boulevard, which is off of TX-288 and Old Spanish Trail.
OSHA did not report specifics surrounding the death of 71-year-old Isaac English. The OSHA summary lists an alleged repeat violation relating to a company’s duty to report an accident within 24 hours of it occurring. It’s unclear if this violation is directly related to this accident, but the summary lists a recommended penalty of over $100,000.

Just to be clear, I cannot confirm anything alleged in the OSHA summary on this accident or any alleged rule violations by the victim’s employer. However, seeing an employer accused of not immediately reporting an accident raises familiar red flags to me. I’ve actually had work fatality cases where the gap in time between an accident happening and an employer reporting it was actually used to try to cover up and destroy vital evidence. I can’t say if that happened here, but that’s exactly why it’s always important to investigate deadly work accidents as soon as possible. There could be more serious issues to address than just an employer’s alleged lack of initiative.
