Pecos, TX — April 26, 2024, Heriberto Elizondo was killed following a work accident at around 10:00 a.m. off US Highway 285.

OSHA officials said that the accident happened near Mentone and the Highway 302 split.

According to investigators, 21-year-old Heriberto Elizondo was working to move a frack tank at the time. While doing so, a catwalk somehow fell, striking Heriberto Elizondo. No other injuries were reported.

OSHA alleged three “serious” violations and listed a recommended penalty in their report. No further information is available.

When it comes to fatal work accidents in Texas, OSHA reports rarely tell the whole story. In fact, even if they confirmed multiple serious violations, that wouldn’t necessarily make things open-and-shut. Despite what many think, there isn’t always an automatic process for getting a worker’s family benefits or even answers after an accident. That can take far more involved independent investigations and navigating a serious legal maze to accomplish. Perhaps more happened behind the scenes here to ensure this person’s loved ones got a fair outcome. My point is that people should never take that sort of thing for granted.
