How Did This Accident Occur?

It’s unfortunately common for OSHA reports to be as lacking in clear answers as this one is. What can surprise people is when their efforts behind the scenes are equally sparse. I speak to families all the time weeks and months following a deadly workplace accident who are still asking the same questions they had on day one.

Considering families in these situations are left to navigate the most complex work fatality laws in the whole country, it’s important they get answers as soon as possible. It’s just that getting those answers often requires independent investigations from professionals who can prioritize the needs of the family rather than their own narrow focus, like OSHA does. Hopefully this accident is in the right hands so that happens and so there can be a clear path forward for a family who lost their loved one.
