While allegations of inattention aren’t confirmed right now, people might make the mistake of thinking things are open-and-shut if authorities do confirm distraction of some kind. However, investigations need to delve beyond what and figure out why the accident happened.

For example, a lot of accidents can happen as the end result of repeated negligent behavior behind the scenes. I recall one case from a while back where a driver was texting and driving. That alone is reckless, but the reason behind was even more concerning. His supervisor essentially hounded drivers all day, and if they didn’t respond immediately, they risked getting fired. Inevitably, this led to drivers trying to appease their unreasonable boss behind the wheel, and it led to a serious accident.

I’m not saying that happened here, but that’s the reason thorough investigations are so important. Maybe this all was the result of a momentary lapse in judgment. Or, maybe it was the end result of a dangerous environment that had been going on already. Accountability means ensuring all those whose actions contributed to the accident answer for them. Are those steps being taken here?
