Decatur, TX — December 10, 2024, Charles Teague was killed as the result of a forklift accident that happened around 1:00 p.m. along US 380 Business.
Preliminary information from an OSHA inspection detail says the accident happened at a business off E. Business 380 and US 380.
According to reports, 49-year-old Charles Teague was operating a forklift at the time. In events which aren’t clear, the forklift overturned on top of Charles Teague. Charles Teague sustained fatal injuries due to the accident.
The cause of the accident remains unconfirmed, but OSHA listed eight alleged violations, seven of which were labeled “serious.” OSHA recommended a penalty. Further information remains unclear at this time.

With OSHA alleging multiple serious violations here, that raises some red flags about the safety at this site and with the victim’s employer. However, allegations and even penalties from OSHA aren’t the beginning and end of fatal work accidents in Texas.
Simply put, we have the most complex work fatality laws of any other state in the country. There isn’t always a guaranteed path for a worker’s loved ones to get a proper resolution following an accident. It’s often up to them to pursue benefits and accountability on their own. Since OSHA has their own priorities to focus on, getting the evidence for those things typically requires secondary independent investigations.
It’s possible steps were already taken here to see the victim’s family had a clear path toward a resolution. However, it’s important for folks to understand that an OSHA report—regardless of what serious violations are found—doesn’t guarantee that.
