Montgomery County, TX — July 30, 2025, Carlos Perez Mendez was killed as the result of a work fatality accident that happened off Riva Row.
The Occupational Safety and Health Administration released few details surrounding the accident. The incident reportedly occurred at 22 Riva Row, just off the corner of Woodlands Parkway and Six Pines Drive.
The factors surrounding the accident are unavailable at this time. OSHA did not say if they’re considering citations. The case status is listed as “open.”

One can only hope that the victim’s loved ones have been able to get more answers behind the scenes than what OSHA has released so far. What might surprise people, though, is families are often just as in the dark as the public is following a fatal work accident.
Simply put, OSHA’s priority is to look for violations and issue citations. They don’t necessarily go out of their way to look out for the needs of a worker’s family. Considering Texas has the most complex work fatality laws of any other state in the country, this can leave them to face significant hurdles on their own unless independent professionals step in to fill in the gaps.
Ultimately, OSHA may conclude their reports here and come up with a more clear explanation of what they believe happened here. But families shouldn’t have to wait months for reports which might not even tell the whole story. I don’t see any reason right now why this incident would be any kind of exception.
