Accident Report
Fort Wayne, IN — June 3, 2026, Bradley Maller and a teenage boy were killed in a truck accident just before 2 p.m. on Interstate 69.
Authorities said a southbound semi-truck collided with two cars that were stopped in traffic in a construction zone near mile marker 295. It also hit another semi-truck, causing a chain reaction that damaged another car and another semi-truck.
The drivers of the first two cars — 70-year-old Kokomo resident Bradley Dale Maller and a 15-year-old boy — were pronounced dead at the scene of the crash south of Interstate 469, according to authorities.
The driver of the first truck was hospitalized with unspecified injuries, authorities said.
Authorities have not released any additional information about the Allen County crash at this time. The accident is still under investigation.
How Did This Accident Occur?
Was there not adequate warning signs to alert drivers to the construction zone? Was the truck driver distracted, perhaps by a cell phone? Did the truck have an issue with its brakes or steering mechanism? Without addressing these questions, authorities won’t be able to figure out why this crash happened.
I expect authorities to pore over physical evidence, witness statements, data from the vehicles and any available video footage as they work to construct this accident. Cell phone records could show what each driver was doing at the time, but an outside expert likely will be needed if investigators suspect a mechanical problem contributed to this crash.
The vehicles should be inspected in a laboratory setting. Most law enforcement agencies don’t have the time or training for that kind of forensic examination, in my experience, but the victims’ families deserve to know as much as possible about what happened on I-69.
