Accident Report

November 13, 2025, Anthony Alcazar was killed as the result of a work accident at the Hillard Center Selena Auditorium.

Authorities said that the accident took place inside the Selena Auditorium, located at 1901 N. Shoreline Boulevard.

It appears that 63-year-old Anthony Alcazar was on a catwalk inside the auditorium. Somehow, Anthony Alcazar fell from the catwalk and sustained fatal injuries.

The preliminary online OSHA report does not list any recommended penalties or citations at this time. Investigations are ongoing.

How Did This Accident Occur?

In my experience, a fatal fall at a workplace is often the result of either a lack of fall prevention equipment or workers being forced to use fall prevention equipment which is worn and needing to be replaced. It’s possible, of course, that something less common happened here. For example, a safety harness may have had a defect, leading to a fall despite the victim taking proper precautions.

A lot of folks think all of that will simply come out in time. OSHA will do their investigations and get the family the answers they deserve. In reality, OSHA tends to focus on their own priorities, and families are often left to look for answers on their own. So whether this was just an unavoidable accident, problems regarding safety enforcement, or dangerous working conditions from the top down, it might take more thorough independent inspections to fill in all the blanks and make sure the family gets the full story. Considering Texas work fatality law can be quite the legal maze, the sooner those steps are taken, the better.
