How Did This Accident Occur?

I find that a lot of people assume work accidents only happen when a worker makes a mistake. However, all kinds of things can go wrong on a job site—especially in the oil and gas industry.

Sometimes, workers aren’t provided with proper safety equipment. Some employers train workers poorly or to do things which are unnecessarily risky for the sake of time. Sometimes, equipment fails due to poor maintenance or even unforeseeable defects at the manufacturing level. It’s even possible a worker from a completely separate company was on site and made a deadly mistake.

At the end of the day, these possibilities all need attention if a worker’s family is going to get the answers they deserve. If OSHA has been open behind the scenes with the victim’s family, that would be good to hear. However, most of the families I’ve worked with on fatal work accident cases brought us in because they felt that authorities weren’t keeping them in the loop or focusing on their needs. I know if I lost a loved one to a workplace accident, I wouldn’t want to wait around for answers, either.

If anyone happens to work in this area, were you surprised to hear there was a fatal accident? Or did you see this sort of thing coming?
