East Providence, RI — March 4, 2025, 14 people were injured in a truck accident at about 9 a.m. on westbound Interstate 195.
Authorities said a semi-truck crashed into 12 other vehicles in morning traffic.

One woman suffered serious injuries in the crash, while 13 others, including the truck driver, were hospitalized with minor injuries, according to authorities. None of the injured people have been identified at this time.
The truck driver has been charged with driving to endanger, authorities said.
Authorities have not released any additional information about the crash at this time. The accident is still under investigation.

When an 18-wheeler crashes into a dozen vehicles during morning traffic, it’s clear that something went seriously wrong. While the truck driver has already been charged, the key question investigators need to answer is why this happened and whether there were deeper issues that contributed to the crash.
One possibility is that the truck driver was distracted, fatigued or driving too fast for traffic conditions. Trucks require far more stopping distance than smaller vehicles, and experienced drivers know to anticipate slowdowns, especially during morning rush hour. If this driver failed to stop in time, was it due to negligence, or were there other factors at play? Investigators should be looking at cell phone records, in-cab camera footage and electronic logging device (ELD) data to determine if distraction or fatigue played a role.
Beyond the driver’s actions, the truck’s condition also needs scrutiny. Were the brakes properly maintained? Was the truck overloaded or unbalanced, making it harder to stop? I’ve handled cases where poor maintenance led to catastrophic failures that a driver couldn’t correct, and those issues often stem from cost-cutting decisions by the trucking company.
It’s also worth considering whether the company itself contributed to the crash. Was this driver on a tight schedule that encouraged risky driving? Had they been on the road too long without adequate rest? These are common issues in the trucking industry, and they often go overlooked when authorities focus only on the driver instead of the bigger picture.
While the charges against the truck driver suggest clear wrongdoing, this crash may not be just one person’s mistake. A proper investigation should look beyond the driver and into company policies, vehicle maintenance and other factors that may have set the stage for this disaster. If those questions aren’t asked, then the real lessons from this crash may never come to light.
