San Antonio, TX — October 24, 2025, Teresa Dominguez was killed as the result of a work accident that happened at an HEB warehouse.
According to preliminary reports, the accident happened in the 5700 block of Rittiman Road, east of Loop 410.

Investigators said that 27-year-old Teresa Dominguez was working at the warehouse at the time. While initial reports say there was a “medical episode,” further reports say an incident may have occurred involving a forklift that led to lower body injuries.
Ultimately, Teresa Dominguez succumbed to her injuries. There were no other reported injuries. At this time, a cause of the accident remains unconfirmed.

This is a good lesson to always read past the headlines. The news reports I saw initially suggested this was some kind of “medical episode.” That usually suggests a condition of some kind, as opposed to injuries caused by an accident. Now, reports suggest this may have been a forklift accident, which can raise far more serious challenges for the victim’s loved ones.
I can only hope efforts to investigate this accident aren’t as scattered as the reporting on it has been. Families deserve to know the death of a loved one is being taken seriously and handled carefully. The spotty early reporting has me wondering whether or not authorities are really giving this accident the attention it deserves.
