Dallas, TX — June 17, 2022, Severo Cardenas was killed as the result of an accident where he was electrocuted on the job.
Investigators with OSHA reported that the incident happened at around 4:30 p.m. 32-year-old Severo Cardenas was doing electrical work at the time and apparently did not use insulated stripper cutter, according to investigators. While working, Cardenas was shocked and sustained injuries which ultimately proved fatal.

In their summary, OSHA recommended potential fines for violations concerning allowing a worker to operate where they could be electrocuted without taking proper precautions to prevent it as well as issues regarding “recording and reporting occupational injuries and illnesses.” To be clear, there would need to be more specific information available to confirm if these violations occurred and what exactly led to those accusations.
But with officials saying an employer wasn’t ensuring a worker was safe while working, it only makes me more suspicious of the claims they didn’t properly report a worker’s injuries to the proper authorities. Was it just negligence? Did it slip their mind due to the stress of the situation? Or could it be they were trying to get their story sorted out before alerting authorities who might then hold them accountable for their mistakes?
Frankly, I haven’t seen much information about this incident despite the concerning nature of the accusations made by OSHA against the victim’s employer. Would more thorough investigations show there are serious issues yet to be brought to light? Are other employees still in danger? If nothing else, do the victim’s loved ones know they’re getting the whole story?
