But time and again, I’ve seen that serious crashes are rarely the result of a single factor. To fully understand what happened and ensure that those affected get the answers they deserve, there are three critical questions that should be asked.

First, did the authorities conduct a thorough investigation? Crashes that happen around 2:40 a.m., like this one on Jim Miller Road, present specific challenges—low visibility, limited traffic, and often few witnesses. A complete investigation should include accident reconstruction, an assessment of roadway conditions, and a close look at vehicle dynamics. But the unfortunate reality is that many departments don’t have the training, equipment, or time to perform this level of analysis, particularly for single-vehicle incidents. That’s why it’s often necessary for an independent investigator to step in and make sure nothing important has been overlooked.
