Bexar County, TX — September 30, 2025, Frank Ramirez Perez was killed as the result of a work accident around 5:00 a.m. off I-37.
According to initial details about the accident, it took place south of San Antonio at 19910 Interstate 37 South.

Authorities said that 79-year-old Frank Ramirez Perez was at Palm Harbor Homes at the time. While outside of his truck opening a gate, the truck began to roll. Frank Ramirez Perez tried to stop the truck, but it ultimately ran him over.
Due to the accident, Frank Ramirez Perez was killed. No other injuries were reported. Additional details are under investigation.

It appears OSHA is looking into this, and for some that makes the situation open-and-shut. In my experience, though, OSHA reports alone are not always enough to get a worker’s loved ones the closure they deserve.
The fact of the matter is OSHA has their own priorities. They are not there to help a worker’s loved ones, and they’re not necessarily trying to find every piece of evidence possible surrounding the accident. Their priorities revolve around finding violations and issuing penalties. That doesn’t always align with the needs of a worker’s family.
As such, it’s sometimes necessary for accidents like this to have another set of eyes looking things over. Not only is an independent investigations likely to get answers quicker than OSHA (who sometimes takes over six months or a year to investigate an accident), they often can secure evidence that OSHA overlooked as a low priority. Whether such efforts would reveal any new information here remains to be seen.
